Why the quiet after treatment is the hardest part

During treatment there is a plan for every week. After it, the plan is mostly waiting, and waiting has no visible activity to it. Many people read that as being dropped. It is not. Surveillance has a specific job: catch a recurrence or a second cancer at a point where treatment still changes the outcome, manage what the treatment left behind, and do both without generating so many false alarms that follow-up becomes its own illness.

That last clause is why the schedule can look thinner than you expected. A test earns its place because finding something with it earlier leads to a better outcome, not merely because it finds things.

What a surveillance plan is actually built from

Nearly every plan is assembled from the same four parts, in proportions that depend on the cancer.

History and physical exam. The most consistent element across every guideline, and the most underrated: it is where you report the symptom that gets imaged.

Imaging, on a schedule or on a symptom. Some cancers get calendar imaging; many get imaged only when something is reported.

Tumor markers, where they have been validated for that disease. Validated is doing heavy work there, and the next section is about it.

Endoscopy or direct inspection, where the surface can be seen. Colonoscopy after colorectal cancer, skin checks after melanoma, laryngoscopy after head and neck cancer.

The same four parts, in three diseases. After curative treatment for colorectal cancer, NCCN advises exam and CEA every 3 to 6 months for two years then every 6 months out to five years, CT of the chest, abdomen, and pelvis at 6 to 12 month intervals for five years, and colonoscopy about one year after resection, repeated at three years and then every five years if it stays clean. After breast cancer, ASCO's 2026 update sets visit frequency by recurrence risk (roughly annual for low risk, every 6 to 12 months for intermediate, every 3 to 6 months for high) with mammography of any remaining breast tissue annually for the first three years and every one to two years after that for selected lower-risk patients aged 50 and over, and explicitly no routine CT, PET, bone scan, or tumor markers without symptoms. After head and neck cancer, NCCN concentrates visits early, every 1 to 3 months in year one, every 2 to 6 months in year two, every 4 to 8 months in years three to five, because that is when recurrence clusters.

Three cancers, three plans that barely resemble each other. If your schedule looks lighter than a neighbor's, the question is not whether you are being neglected. It is which cancer, which stage, which guideline.

Why more scans are not automatically better

Scanning more often should catch things sooner, and sooner should be better. The evidence is more careful. In lymphoma, the Lugano recommendations explicitly discourage routine surveillance scans after a complete remission, particularly in diffuse large B-cell lymphoma and Hodgkin lymphoma. In breast cancer, ASCO repeated the same position in 2026: no routine body imaging for distant metastases in asymptomatic patients. In colorectal cancer, the FACS randomized trial (JAMA 2014, 1,202 patients) found that intensive CEA or CT follow-up did increase the number of recurrences treated surgically with curative intent, but overall mortality did not differ significantly, and the authors concluded that any survival advantage was likely to be small.

Every scan has a false-positive rate, and a false positive in someone who has had cancer is not a minor inconvenience: it is a biopsy, a wait, and weeks of fear. And routine tests are often not what finds the recurrence anyway. In a Swedish cohort of 2,728 breast cancer patients (Sartor et al., Breast Cancer Research and Treatment, 2025), 95% of distant metastases were found because the patient presented with a symptom, not through planned surveillance, while 72% of second primary cancers in the contralateral breast were caught on scheduled mammography. Together they explain the design: keep the test that reliably catches something early, drop the ones that mostly catch what the person would have reported anyway, and make sure the person knows which symptoms not to sit on.

What a rising tumor marker does and does not mean

A marker is a monitoring tool inside a specific disease, not a general cancer detector, and it behaves differently in each one.

CEA rises in plenty of things that are not cancer, including smoking, cirrhosis and other liver disease, pancreatitis, inflammatory bowel disease, and chest infections, which is why reference ranges run higher for smokers. One value above the range is not a recurrence. What matters in colorectal follow-up is a confirmed, sustained rise, which is why a repeat test comes before a scan.

CA-125 after ovarian cancer is the sharpest example of why an earlier answer is not automatically a better one. In the MRC OV05/EORTC 55955 randomized trial (Lancet 2010), starting chemotherapy on a rising CA-125 alone, before any clinical sign of relapse, produced no survival benefit over waiting. It moved the treatment and its side effects earlier without moving the outcome.

PSA after prostate surgery sits at the other end. AUA defines biochemical recurrence after radical prostatectomy as a PSA of 0.2 ng/mL or higher with a confirmatory second value above 0.2 ng/mL, and that threshold triggers a real decision. The half of follow-up that is not about recurrence

Watching for cancer is one column of the plan. The other is what treatment left behind, and it runs on a longer clock. Anthracycline chemotherapy carries a documented risk of cardiotoxicity that can appear years after the last dose. Radiation raises the risk of second cancers in the treated field. Endocrine therapy affects bone density. Some regimens leave lasting neuropathy, fertility effects, or thyroid dysfunction. None of these are found by looking for the original tumor.

This is what a survivorship care plan is for. The Institute of Medicine's 2005 report "From Cancer Patient to Cancer Survivor: Lost in Transition" recommended that every patient finishing treatment receive a written treatment summary plus a plan covering surveillance, late effects, and who owns which part.
